The Department of Labor and Training has instituted the following procedure to help the public obtain public records pursuant to RIGL §§ 38-2-1 et seq. 

  • To reach us by telephone call the above-cited telephone number and speak to a staff attorney. 
  • The regular business hours of the Department are 8:30 AM to 4:00 PM. 
  • The Department may ask you the reason for your request as its regular course of business; however, you are not required to provide the reason you seek the information, and your right to access public records will not depend upon providing reasons. 
  • So that you are provided with the public records you seek expeditiously, the Department asks that you complete the Public Record Request Form, which can be found below.  
  • There are times when the public records you seek are not available at the time of your request. Please be advised that the Access to Public Records Act allows a public body ten (10) business days to respond, which can be extended an additional twenty (20) business days for “good cause.” The Department appreciates your understanding and patience. 
  • If you believe that you have been denied access to public records, you have the right to file a review petition with the Department of Attorney General. If you are still not satisfied, you may file a lawsuit in Superior Court. The Department of Attorney General’s Guide to Open Government may be obtained on its website . 
  • This Department is committed to providing any individual with public records expeditiously and courteously.

Personal Records Release Requests

To request copies of your personal records from the Unemployment, Temporary Disability/Temporary Caregiver Insurance, or Worker's Compensation programs, complete the Personal Records Release Form

Personal Record Release Forms can be submitted in the following ways:

  • In Person: You may complete this document at the DLT, and a Notary will be provided by appointment. Please call 401-462-8877 to schedule an appointment with a notary. If you show up without an appointment, a notary may not be available for you. The address is 1511 Pontiac Avenue, Building 73-3, Cranston, Rhode Island 02920. 
  • Mail: Submit the signed and notarized form via regular mail to 1511 Pontiac Avenue, Building 73-3, Cranston, Rhode Island 02920, c/o Legal Division.
  • Email: Email the signed and notarized form to Marangely Ortiz, Legal Secretary, at Marangely.Ortiz@dlt.ri.gov.
  • Fax: Fax the signed and notarized form to 401-462-8884, c/o Legal Division.
